Insurance Plans
INSURANCE
We are credentialed with most healthcare insurance plans and we will bill your insurance company as a courtsey.
Please supply a valid ID and an updated insurance card at each visit.

You can check to see if our practice is included in your health insurance network by visiting their website on the back of your insurance card. We are credentialed with the following insurances:

Your co-pay or deductible is determined by your insurance plan and benefits. Many insurance plans require that you make a co-payment at the time of service. Patients are responsible for their co-pay and any other unpaid amounts leftover.
During your visit, our staff will be happy to assist you with determining your coverage and your out-of-pocket responsibility. Additionally we offer a sliding fee schedule for patients that meet the income requirements.

PATIENT PORTAL
The Patient Portal is where you can access and edit your personal health information and make partial and full payments.

To get access to the patient portal, call our office and request an email invitation to sign up.
We are happy to file the claim for you, but the balance is your responsibility if your plan does not pay after 30 days.
Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.
PAYMENTS

We accept most major credit and debit cards, cash and checks. You can make payments via the patient portal any time.

Self-Pay
Don't have health insurance? Have a high-deductible plan? Our self-pay prices are a cost-effective solution to that gets you the immediate, quality healthcare assistance you need for non-life-threatening illness and injuries.

Medicare
We accept Medicare patients. As a courtesy, we will also bill your secondary insurance provider directly. You will be asked to sign an Advanced Beneficiary Notice or ABN for services not covered under Medicare. You shall be accountable for covering these services.
